1095-C Form 

This year you will be receiving your 1095-C forms, from our partner NavigateHCR. They will be delivered from the email address NavigateHCRmessaging@navigatehcr.com with the subject line: Receive your 1095 form.

Please follow the instructions on the email and download your 2023 1095-C form. Please note that you are NOT required to include Form 1095-C with your personal income tax filing. However, you should keep Form 1095-C with your personal tax records as the required proof of the health insurance you maintained.
